Correction of Clerical Error and Change of First Name

The local civil registry office also offers services on the correction of clerical errors (CCE) and change of first name. There was a time that these services would go under a tedious process that includes going to court but these can now be availed at the local civil registry.

REPUBLIC ACT 9048 (known as) CORRECTION OF CLERICAL ERROR (CCE) BIRTH, MARRIAGE AND DEATH

Requirements:

  1. Certificate of Live Birth, Marriage and Death (SECPA)

  2. Baptismal Certificate

  3. Secondary and College Transcript of Records

  4. Valid Identification Card

  5. Police Clearance

  6. NBI Clearance

  7. Affidavit of Two (2) Disinterested Persons

STEPS:

  1. Receiving/Review of Petition

  2. Payment of Fees (Treasury Office)

  3. Preparation and Posting (10 days)

  4. Decision of CCE

  5. Submission of Petition (NSO Manila)

  6. Affirmation of Decision

  7. Issuance of Finality

Requirements:

  1. Certificate of Live Birth, Marriage and Death (SECPA)

  2. Baptismal Certificate

  3. Secondary and College Transcript of Records

  4. Valid Identification Card

  5. Police Clearance

  6. NBI Clearance

  7. Affidavit of Two (2) Disinterested Persons

    4. Publication of Petition (2 weeks)

    5. Decision of CFN
